The Nephrology department of Henri Mondor hospital (Paris-East, Creteil) is a multidisciplinary structure, which insures annually 1,143 hospitalizations (206 in intensive care), 120 transplantations, 2,395 sessions in day hospital, 8,013 dialysis sessions and 9,000 outpatient appointments. It receives growing number of international patients for diagnosis assessment and specific therapies including renal replacement and transplantations (20% living donors). Kidney diseases, of immune or non-immune origin (genetic, metabolic, vascular, cancers, etc) are managed. Composite transplantation (kidney/pancreas, kidney/liver, kidney/heart) are performed in collaboration with corresponding departments within the hospital. Our reference center for Idiopathic nephrotic syndrome (INS) is individualized within the Nephrology department. Expertise is mostly focused on glomerular diseases of immune origin including clinical, pathophysiological and therapeutic aspects. We provide for practitioners two official guidebooks, published by the Ministry of Health, describing the management of pediatric and adult INS including diagnosis, treatment and follow-up of patients. We published several booklets dedicated to different categories of patients with INS (children and adult steroid sensitive INS, steroid resistant INS and post transplant recurrence). We have contributed to creation of INS patient Organization (AMSN), which includes several hundred of members and has evolved to become the first contributor for French research in INS (one million Euros over six years). Several associations of patients from European countries (Italian, English, Netherland) have been invited during annual joint-meeting patients/doctors. The clinical activities are complemented by clinical, translational and fundamental researches in the field of INS. Clinical investigations are mostly focused on INS therapy in adults because no consensus exists with respect to duration of steroid therapy or clearly proved strategies for alternative therapies. Several National clinical and translational research programs have been coordinated by the reference center. |
